When I was there for my treatment the office was great and so was the doctor. However, now that I need them to complete an evidence of insurability for a life insurance application, they are telling me they will not complete it, which means I will not be approved for my life insurance. The doctors notes are also incomplete in my file because after three visits he told me nothing was wrong with me and I did not need to return. The files say I was suppose to come back in two weeks. After three visits why would I not go back for a fourth visit if he told me to? Horrible service and if you speak with Susan she has no heart and can careless why you need the form. I guess I was just a file and quick paycheck. Awful service.
