For the past four months, I have been attempting to get the Medical Release form faxed to my primary care doctor and have yet to receive it. This is severely hindering me from getting the much needed medical attention that I require to remain healthy as I can be. I am a Systemic Lupus Erythematosis patient and am in need of my medical treatments in addition to my other ailments. I am reaching out in this manner as my next step is to contact a director/owner/person in charge. I feel that I have been allowed to slip through the cracks within the system. This makes NO sense!!

My community college introduced me to Access to Care, and although it may seem like a lot of paperwork to send over, it is worth for when I go visit my primary doctor and feel secured that my personal wallet will not be primarily affected by it. Also, I get reminders on the mail about updates on the insurance, thank you
