This place is run down. The staff is rude. Had to bring fan from home because it's hott.

STAY AWAY. I would give this place a ZERO if I could. The way they treated my grandmother (80) during her brief stay here was horrific. Not once was my family called to be notified of her condition or status, nor her physical therapy process. Her medicine was given at the WRONG TIME OF DAY AND WRONG DOSAGE. We have the discharge papers to show the inaccuracies. The staff was never even INFORMED that my grandmother had DEMENTIA. She was here for PT from losing her ability to walk from Covid19. How is your staff not told of her medical conditions??? Horizon was informed that without her hearing aids she is DEAF. They knew she needed assistance with them. If she does not have them in, she is disoriented and is mentally exhausted from trying to understand her surroundings. When my family picked her up, the hearing aids were IN A BAG OF CHEX MIX. They were not charged. When my family picked her up on Sunday Feb 14, the pants she was wearing smelled of dry URINE. DONT TAKE YOUR FAMILY HERE IF YOU LOVE THEM.

This place was awful and I didn't stay a full 12 hours. The staff was so rude and dismissive. I had to argue with the nurse to get the medication prescribed to me. They did not give me my antibiotics, which is the reason why I was there. When I tried to address the issues, I could not get ahold of anybody. And for dinner, I was handed a tray with (no joke) 4 chicken nuggets, a scoop of coleslaw, and a 4 oz juice cup. It was the first and worst experience I've had with a care facility. Fortunately, I'll be headed to a more skilled and organized facility. I would not recommend this place to my worst enemy. I also noticed most of the 5 star reviews for this place are not from residents.

The marketing director is really helpful around the facility and is always a team player. He always steps up and helps out others when needed. He is a great asset to the company and the facility. Kudos to Jennifer, the receptionist who is always calm with callers when expressing grievances. Jeanette is also helpful with admissions and makes sure she is thorough with information. Mahogany is also a fantastic CNA, she has a calm demeanor with staff and residents
